If you prefer the hard copy, please call our office at (306) 559-7226 and we will arrange for it to be delivered to your office.

ULTRASOUND REQUISITION VICTORIA EAST RADIOLOGY

The completed requisition can be faxed to (306) 559-7227. After receiving the requisition, we will contact the patient to confirm a date and time for appointment. If you would like to be notified of patient’s appointment time, please include a note on the requisition or call us directly to book the exam.

Please include the following information on the requisition:

  • Referring Doctor info (Name, Signature, Billing Number, Phone and Fax Number)
  • The type of exam requested and relevant clinical history
  • Urgency of appointment
  • Patient Name, Health Number, Date of Birth and Phone Number
  • If report should be CC’d to any other Doctor
Please call our office to book all urgent exams.

Please note: You should receive a finalized report within 24 hours after the exam has been completed. We will call you at the provided number if any results require immediate attention. If we are unable to reach you (ie after hours) we will contact the doctor on call.
